SECOND CHANCE EXAM REQUEST FORM

Enter your name, the name of the class you failed, comments (optional) and click submit. You will be emailed the date and time of the next scheduled second chance exam as confirmation of receipt. These are usually held on a Friday around 4:00 pm, but check the information in your confirmation email. Remember, second chance exams cover the entire course and may include questions which were not covered in class. Also, your second chance exam score will be entered into the instructor's grading rubric, so receiving a passing score on the exam doesn't necessarily mean you will pass the course. The highest final grade that will be recorded for a second chance exam is 70%.
