APPRENTICE REQUEST FOR AN ADDITIONAL ABSENCE

This form should be completed by an apprentice who feels that an extenuating personal circumstance has precluded him/her from attending school, and would like to request that the Committee allow them an additional absence(s).

In the event that the request is granted, the apprentice is responsible for making up any missed work and tests per Policy, and must still pass the class to receive credit.

Please attach any pertinent documentation that relates to this request using the attachment button at the top of this page OR as an attachment via email to tschulz@electricaltc.org and make sure to note this below in the "Describe reason for request" spot.
