    OHIO STATE FIRE ALARM TEST INFORMATION

    INSTRUCTIONS FOR JOURNEYMAN OBTAINING A LICENSE

    As of 6/21/2018, the Ohio State Fire Marshal changed the procedure for obtaining a Fire Alarm License.  Test are now administered every weekday at PSI Services, 4000 Executive Park Dr., Sharonville, OH  45241.  There is a two-step process you must follow:

    Step 1:                Applying for the Test          $35.00

    To apply directly with the Ohio Fire Marshal's office:

    Reference Materials & Filing Instructions For Fire Protection Certification Exams These pages include reference materials and Important filing instructions.
    Page 3 Application for Individual Fire Protection Certification Complete all applicable fields in the "Individual Information" section and sign.  Be sure your first and last name match your ID EXACTLY!
    Page 4 Irrevocable Consent to Legal Service ONLY COMPLETE THIS PAGE IF YOU DO NOT LIVE IN OHIO...Page 4 must be completed in its entirety and NOTARIZED. Page 4 can be notarized at the JATC office if you personally bring the form in. 
    Application Submission Instructions Return completed pages 3 and 4 (if applicable)

    Either mail application to:  Division of State Fire Marshal Testing & Registration Bureau, 8895 E. Main St., Reynoldsburg, OH  43068

    Make payable to: Treasurer, State of Ohio                           (Fee is non-refundable)

    To apply through the JATC:

    Either drop off or Mail to:  Cincinnati Electrical Training Center 5455 Glenway Ave., Cincinnati, OH  45238

    Payment Instructions Pay by cash, check or credit card

    To pay by cash you must stop into the JATC office. 

    To pay by credit card click here and select Fire Alarm for the fee and class type options.

    Make check payable to: JATC and either mail to the address above or drop off to the office. 

    • Once the OSFM receives your payment and application, they will send you a Candidate ID by email.  You can now schedule the test.

    Step 2:                Schedule the Test          $40.00

    • The JATC will be unable to schedule your test for you unless you apply as part of a class.  You can schedule the test by phone or online:
      • By Phone: Call 1-800-733-9267.  Have your Candidate ID and credit card ready to pay the $40.00 fee.
      • Online: Follow the below instructions:

    Online Test Registration Instructions:

    1. Go to psiexams.com
    2. Create account (email address, first and last name - must match exactly with app)
    3. Check box "Check here to attempt to locate existing records for you in the system"
    4. Select exam and Candidate ID that came in email from SFM
    5. Enter zip code and list of testing sites will appear
    6. Dates will appear for selected site
    7. Follow instructions to select date and time
    8. Follow instructions to pay for test

    • You must schedule, take, and pass the Fire Alarm test within 90 days of the date your Candidate ID is issued.  The test may be scheduled multiple times but the fee will apply each time you schedule a test.
