Change of Personal Information Since NW State requires their own form for Change of Information in order to make things simpiliar we have now combined our change of information form with theirs and because of this you will no longer be able to update this information with the JATC online. Please click on the link below to print out the combined form and complete BOTH the front and back of the PDF. Once complete you can either mail, fax or submit to our office in person.
